Patient information: This healthy and relatively fit female in her mid-fifties wished to improve her abdominal wall and breasts, a Mommy Makeover. She had had one child but did not have sufficient skin laxity for a tummy tuck. Her fatty layer was up to two inches thick. She also disliked her breasts, dome 25 years after implant insertion. Her breasts were rock hard and droopy, and a recent mammogram showed that the scar tissue around her old silicone implants was calcified and the implants were broken. She sought consultation with Dr. Juris Bunkis of Orange County Plastic Surgery in Newport Beach, California. Dr. Bunkis removed her old, ruptured breast implants and scar tissue, inserted new Mentor, textured silicone, 250 cc implants and did an anchor technique breast lift. Patient information: This otherwise healthy female in her mid sixties was seen by Dr. Juris Bunkis of Orange County Plastic Surgery in Newport Beach, California for concerns about her painful, rock hard breasts. She had had silicone implants inserted almost 40 years earlier and had been told by her physician for years that mammograms have shown her subglandular (on top of the pectoral muscles) implants were ruptured and the surrounding scar tissue was severely calcified. Dr. Bunkis concurred with the family physician that the old implants and scar tissue should be removed and gave the patient to option of leaving her implants out or inserting new ones. The patient chose to have slightly larger silicone implants inserted. Dr. Bunkis was able to excise the entire capsule (scar tissue surrounding the implant) intact and to remove it and the implants without spilling any silicone in the patient. He made new pockets under her pectoral muscles and inserted 300 cc, Mentor, moderate profile, smooth gel silicone implants. She was very pleased with her results and remains completely soft five years later.

Patient information: This healthy female in her mid sixties, had developed severe capsular contractures 25 years after insertion of silicone breast implants. Her breasts were now rock hard and painful and she was “now over them”. She was in a stable relationship and wished to have her implants removed. She saw Dr. Juris Bunkis of Orange County Plastic Surgery in Newport Beach, California, told him she wished to have her implants removed and that she was worried that she would be very droopy afterward. A breast lift was discussed but Dr. Bunkis explained that it was impossible in advance to predict how much the skin would shrink and that it was best to first remove the implants and to do the lift later if it was desirable to do so. The patient concurred Dr. Bunkis performed a capsulectomy (removal of scar tissue around the implants – if this is not done, the pocket around the implant might not obliterate and could fill with fluid in the future) and removed her implants. A year later, her skin had retracted and she was quite pleased with her result and has not required a breast lift. She was much smaller than before the implant removal and had to buy some new clothes but was very happy with the results because her breasts now felt natural and did not hurt.
