Last weekend we visited Newburyport, Massachusetts and were charmed by its beauty and history. Newburyport, nestled on the picturesque shores of the Atlantic Ocean just south of the New Hampshire border, is not only a scenic coastal town but also a hub of rich historical significance. Stepping into Newburyport is like entering a living museum, where every corner whispers tales of its storied past.
During our visit, we had the pleasure of staying at the esteemed Garrison Inn, a charming establishment named after the renowned abolitionist William Lloyd Garrison. Garrison, a prominent figure in American history, dedicated his life to the fight against slavery and advocated passionately for women’s right to vote. The inn pays homage to his legacy, providing guests with a unique opportunity to immerse themselves in the historical fabric of the town.
Wandering through the streets of Newburyport, one can’t help but be enchanted by its colonial architecture, quaint shops, and cobblestone pathways. The town’s maritime heritage is evident in its bustling waterfront, which includes a maritime museum.
Newburyport’s historical significance extends beyond its architectural charm. It was a thriving center of trade during the 18th and 19th centuries, playing a crucial role in the region’s economy. Today, visitors can explore its rich maritime history at the Custom House Maritime Museum, which offers fascinating insights into the town’s seafaring past.

Normally, we focus on procedures we do frequently but today I will discuss a relatively rare procedure – rare because it is rather complex, required multiple stages and is requested infrequently. Today I will discuss forehead reduction surgery, also known as hairline lowering surgery. It is a cosmetic procedure aimed at reducing the size of the forehead and lowering the hairline to achieve a more proportionate facial appearance. One method of performing this surgery involves the use of tissue expanders.
During the initial stage of the procedure, a tissue expander is inserted beneath the hair-bearing portion of the scalp. This expander is gradually filled with saline solution over the course of approximately three months. The gradual expansion of the tissue allows the scalp to stretch and accommodate the forthcoming changes.
After the tissue has been adequately expanded, the second stage of the procedure is performed. Both take place as outpatient procedures under a light general anesthetic. The tissue expander is removed, and the upper portion of the forehead is excised. By removing this excess skin and tissue, the hairline can be advanced downward, effectively reducing the size of the forehead and creating a more aesthetically pleasing facial profile.
The advancement of the hairline is carefully planned to ensure natural-looking results and optimal symmetry. The incisions are meticulously closed, and post-operative care instructions are provided to promote proper healing. The final scar rests against the frontal hairline and will be barely perceptible in a light skinned patient but I would caution someone whose skin is darker or anyone with a history or producing keloids not to have this procedure performed.
Forehead reduction surgery is often considered a two-stage process to achieve the desired outcome while minimizing potential complications. By utilizing tissue expanders, surgeons can gradually prepare the scalp for the necessary adjustments, ultimately resulting in a more successful and satisfying outcome for the patient.
Overall, forehead reduction surgery with tissue expansion offers patients the opportunity to achieve their desired aesthetic goals through a carefully planned and executed series of procedures. This procedure can produce remarkable results for those patients bothered by a high hairline.

To begin with, an incision was made across her forehead along a line where she wished to have the final hairline, and the expander was inserted above this line, beneath the hair bearing scalp. The expanders have a valve that a surgeon can palpate through the skin, and the patient comes in weekly for saline injection into this port to expand it. This is what she looked like three months after the tissue expander was inserted, fully expanded, prior to removal of the expander, excision of the skin between the incision and the hairline and moving the hairline down to a more average position (Surgery performed by Dr. Juris Bunkis)
